09-19-95-A PENNSYLVANIA PESTICIDE HYPERSENSITIVITY REGISTRY FACT SHEET WHAT IS THE REGISTRY? The registry, which is maintained by the Pennsylvania Department of Agriculture (PDA), is a list of people who have been verified by a physician to be excessively or abnormally sensitive to pesticides. Commercial and public pesticide applicators must contact anyone on the registry whose listed location is within 500 feet of a pesticide application site. By having prior notification the sensitive individual can take appropriate precautions. PDA will send, by mail, the registry to each licensed commercial/public pesticide application business. The registry will be sent on or before the effective dates of March 1, June 1, September 1, and December 1 of each year. A person will not be considered included on the registry unless their name appears on the current registry. A request for listing on the registry must be received by PDA at least two months before an effective date. What are the notification requirements? Notification is mandatory and must be made not less than 12 hours and not more than 72 hours prior to the pesticide application. Notification requirements are met if the information is placed on a telephone answering device, or information is given to an adult contacted by dialing any of the listed telephone numbers. If notification cannot be made after at least two telephone contact attempts, written notification can be placed on the front door of the listed location. HOW DO I GET ON THE REGISTRY? Fill out part I of the application. Be sure to include the county, township, borough or municipality of each location. Primary location is your legal residence. Secondary locations) is limited to @ of the following: place of employment; school; vacation home. Also, you must provide a daytime, nighttime and alternate phone number where notification information can be conveyed. See your physician about filling out part II. Mail the completed application to the Department of Agriculture (PDA), Bureau of Plant Industry, 2301 N. Cameron Street, Harrisburg, PA 17110-9408; (717) 787-4843. PDA will notify you of placement on the registry. Because the registry is a public record, the confidentiality of registry information cannot be assured. In order to remain on the registry, you must notify the Department annually during January (a renewal form will be provided by PDA). Additional medical verification will not be required for annual renewal. WHAT IF I AM NOT NOTIFIED OF A PESTICIDE APPLICATION? If you are on the registry and a pesticide application was made by a commercial or public applicator within 500 feet of your property, we suggest the following: Personally contact the pesticide applicator. Explain that you are on the registry and need to be notified of pesticide applications. Contact the PDA regional office in your area (see map on reverse) and ask to speak with an Agronomic Products Inspector. They will record the information regarding your complaint and assist in the appropriate manner. It would be helpful if you could provide the inspector with the "BU" number displayed on both sides of the application service vehicle and any application site information.
